There are probably a few things you need before seeing us:
1. check with your insurance or primary doctor to see if you need a referral, most PPO plans do not require it.  Likely an HMO plan does, we ask that you not assume to avoid problems and being able to see you.
2. arrive 15 minutes early to fill out paper work unless you have downloaded the forms on our web site then 5-10 minutes early is adequate.
3. bring your insurance card as we will need to make a copy of it.
5. due to contracts we have with your insurance it is likely that we will need to collect a co-pay from you, this is not in every case, but most.  We accept cash, check and most credit cards.  We are not a bank and don't have the ability to cash large bills, we carry very little money in the office.
6. bring in any x-rays, MRI or CT exams/reports that you may have.
7. if you think your problem is related to the way you walk or previous arch supports/orthotics then bring them in.
8. this is not necessarily an all inclusive list, but should remind you of some helpful things to make your visit more efficient for you and easier for us to help you.

As best possible, fill these forms out prior to your visit.  We have these forms also in the office, but request you come 15 minutes early to fill them out if you choose to do it this way, otherwise the forms below should save you some time in our office.  **Please note that due to government regulations there are specific questions we are required to have on the forms, whether you choose to answer them is up to you.
